Dr Gaydos does not participate with Health Insurance or Medicare. Payment is made at the time of your visit.
Medicare does not provide reimbursement for services rendered.
Most private insurance patients receive 50 % or more reimbursement from their Insurer. Exceptions are "managed care" plans.
Auto Accident Claims will reimburse you generally 100% of your paid Claim.
Dr Gaydos does not accept MediCal or Worker's Compensation Insurances.
If you have questions regarding reimbursement from your health insurance, please contact your Insurance Company. I have listed the Procedure Codes (below) that you may reference to your Insurer. These codes are common and are not esoteric. The diagnoses and code sets billed are Medically Necessary in all cases.
Dr Gaydos can electronically submit your paid claim to your Insurance Carrier as long as you provide this information during your Portal set-up, or text-email a picture of the Front-Reverse of your Insurance Card with the NAME and DOB of the Subscriber before your visit.
This service is fast and efficient, and saves you time.
For your convenience, the office accepts Credit, Business, HSA/FSA and Debit cards of all Types, and from all Countries.
A DISCOUNT is provided for the use of Cash, Personal Check or ZELLE.
IN ORDER to give you the most time with Dr Gaydos, please have your form of payment ready prior to the start of your visit (have your check written, or the Card on the desk before we begin).
Less Time for Administration means More Time for your Appointment.

NEW PATIENT FEES:
There are a variety of New Patient office visit types for you to choose from, depending upon your needs.
New Patient Complex ($490 - 120 minutes): For patients of all ages who have a lot of medical concerns and medical history. Meeting for two hours allows a Full History, Examination and Osteopathic Treatment, and Q & A. You will receive a Summary of my recommendations afterwards. (CPT Codes: 99205/98929).
New Patient Adult Simple ($360 - 60 Minutes): Patients with an organized and Straight-Forward History (5-10 minutes) who want an Osteopathic Hands-on Manipulation with limited Diet and Lifestyle advice. (CPT Codes: 99203/98928).
New Patient Child (5 years and younger - $240 - 60 minutes): Children with a Straight-Forward History who want an Osteopathic Hands-On Manipulation with limited Diet and Lifestyle advice (CPT Codes: 99203/98927).
You may use these CPT codes to verify that your Insurer will consider reimbursing you for a New Patient visit.
*There is a $10 New Patient Discount if the visit is paid in Cash, Personal Check, ZELLE (use my email username) or paying with VENMO (for fastest processing ask to SCAN my code in-office using your phone, or use "@drgaydos" as my username when you make payment).

Established Patient: ($210 - 60 minutes): This provides a History and Hands-On Osteopathic Manipulation (99213/98928).
Established Child (5 years and younger - $190 - 60 minutes). Provides a History and Hands-On Osteopathic Manipulation (99213/98927).
ESTABLISHED PATIENT REMOTE / LIVE CONSULT FEES:
Established Patient Consult (any Age - $51.25 per 15 minutes ): Consultation regarding Diet, Exercises, Lab or Radiology Reports, Glasses, Electronics, Supplements, Medications, Orthotic Check, Dental Appliances, and more. Provides simple compatibility testing with Q & A. The Established Patient Consult may be provided by Live Attendance or as a Telemedicine visit, if requested.
There is a $50.00 fee added for in-office Visual screenings for New or Established Patients.
You may use these CPT codes to verify that your Insurer will reimburse you for an Established Patient visit.
*There is a $5.00 Established Patient Discount if the visit is paid in Cash, Check, Zelle, or paying with VENMO (for fastest processing ask to SCAN my code in-office using your phone, or use "@drgaydos" as my username when you make payment).

CANCELLATION POLICY: There is a "Two Business Day By Noon" cancellation agreement, or you may be financially responsible for the visit if we are unable to fill that time slot.
Business Days are Monday through Friday, and EXCLUDE weekends and Federal Holidays.
EXAMPLES: To cancel a Monday appointment, please call Thursday by Noon.
To cancel a Tuesday appointment, please call Friday by Noon.
To cancel a Thursday appointment, please call Tuesday by Noon.

Please bring your completed paperwork for the first visit. Come early if it is not completed, as I am UNABLE TO EXTEND YOUR VISIT TIME while you fill it out. I will put the paperwork on the GREEN clipboard as you enter the waiting area.
Please arrive for each office visit 5-15 minutes ahead of schedule, so that you have time to sit and "collect yourself" prior to the visit.
Wear comfortable clothing that allows you to move and breath. We have a spacious bathroom if you prefer to change before your visit.
Please come as hydrated as possible - there is complimentary hot water and tea for your enjoyment.
The Bay Area Wellness Building is a SCENT-FREE office space. Please DO NOT WEAR FRAGRANCES in the office.

It is my goal to always provide you with medical care that is specific to you and to your circumstances. I do not follow a formula or equation in my care. It is my highest priority to understand you as a Whole Person, to treat you as a Whole Person, so that the Injured Part may begin to Heal under the influence of the Whole Person, as is Nature's dynamic. DR GAYDOS DOES NOT SHARE YOUR PRIVATE HEALTH INFORMATION (PHI) with any person, agency or organization, unless the organization sends a Signed Release. If you wish to share your records, they are available on your Portal on demand.
We welcome that you remain available to take calls on your cell phone during your office visit, but request that you take lengthy conversations outside and away from the building, to respect the quiet space of the office, and the other patients and practitioners.
